Fees

A processing fee of 500 yen per request will be charged for notifications of purpose of use and disclosures. Payment must be made by postal money order and enclosed with the request documents.

When Requests May Be Declined

  • If the submitted documents contain omissions or errors.
  • If the required fee is not paid.
  • If we are unable to verify your identity.
  • If a representative submits the request but cannot provide proof of authorization.
  • If the request does not pertain to "disclosable personal information".
